《猛將傳》是以增加新要素與新模式的方式所製作推出的《真‧三國無雙》系列延伸作品，遊戲是以本傳為基礎，並加入包括新關卡、新武器、新道具、新遊戲模式等內容。
本次推出的《真‧三國無雙 4 猛將傳》，將加入新的「外傳模式」，玩家將化身為三國武將，並為了達成被賦予的任務，而奔馳在千軍萬馬敵我交錯的戰場上。外傳模式共準備了 18 組劇本，其中包括「漢水之戰」、「蕸萌關之戰」及「陽平關之戰」等，都是無雙系列作中全新未公開過的戰場劇本。
之外還包括左慈、張角及龐統等武將為了爭奪傳說中道術書的「道術書爭奪戰」，甚至包括女性角色們一邊守護夫婿一邊作戰的「郎君炫耀戰」等等，玩家將可嘗試以熟悉的角色，來進行全新登場的「外傳模式」。在第一次遊戲時，可以在每個選定的劇本中指定武將、戰鬥的流程與事件，而過關時的訊息內容會因選擇的武將而產生變化，同一劇本也將會發展出完全不同的遊戲樂趣。
在難易度設定方面，除了以往的「簡單」、「普通」、「困難」與「修羅」模式之外，本作還新追加更簡單的「入門」模式，可讓更多的玩家一同享受《真‧三國無雙》的遊戲樂趣。

【漢水之戰】 概要
針對趙雲及黃忠的遊戲劇本。主要敘述趙雲、黃忠與曹操大軍之間的漢水之戰！在本陣之前阻止魏國大軍，一騎當千、一身是膽的趙雲，以及抄小徑奇襲魏軍本陣的黃忠，兩人的傳奇事蹟傳頌千古。
【蕸萌關之戰】 概要
針對張飛、諸葛亮、魏延的劇本，描述張魯軍因受劉璋所託而與劉備發生的戰役。有著與馬超發生激烈戰鬥而倍感興奮的張飛、攔腰突襲的魏延，以及運籌帷幄、邊掌握敵軍並能四處活躍的諸葛亮等，本劇本可以享受到各種不同的情境。
【陽平關之戰】 概要
劇本的主角為曹操、夏侯淵、張郃及曹仁，描述以一統天下為志業的曹操和阻擋在其面前的五斗米道張魯一派之戰鬥。率領本隊進軍的曹操、打算從小路迂迴奇襲的夏侯淵、還有幫助曹操進軍的張郃與曹仁，可用各種立場角度來享受滿是機關的陽平關之戰。
【道術書爭奪戰】 概要
身為道教始祖且擁有無上力量的太上老君，打算將自己撰寫的「奇蹟道術書」送給他人。自認為最有資格的左慈、張角及龐統三人，從此展開了太上老君的試煉之戰。
【郎君炫耀戰】 概要
由於左慈的提議—「將神仙玉璽奉獻給最好的丈夫」！孫策與大喬、周瑜與小喬、曹丕與甄姬還有諸葛亮與月英等四對夫妻，集結到了左慈的城池。為了證明自己的丈夫是最棒的、自己的丈夫才是最好，四名偉大的女性於是展開了激烈對抗。
